The evidence explains what is supported. The toolkit shows what leaders do next.

A direct pathway through the Evidence-Based PFAC Summary, AMA 11 source layer, evidence matrix, benchmark context, and working leadership tools.

Evidence-Based PFAC Summary

The core argument in one place.

This APE synthesizes the CMS Patient Safety Structural Measure, Domain 5: Patient and Family Engagement, current PFAC evidence, and the approved applied-practice deliverables into a practical leadership product.

The project does not assume that establishing a PFAC causes better outcomes. It focuses on the operating conditions leaders can assess and manage: sponsorship, governance, representation, meaningful decision involvement, measurement, action closure, and report-back.

AMA 11 source layer

Peer-reviewed research, federal guidance, implementation resources, public benchmark sources, and story resources are separated for clean traceability.

Dartmouth Health assessment

Supported findings and open questions remain clearly separated; no score is assigned without evidence.

Evidence provenance

A defined baseline with a visible source trail.

A 51-record Zotero export anchors the evidence review.

The public bibliography identifies additional sources used by the applied project. New sources are classified and recorded rather than silently folded into the baseline.

Deduplication, retraction checks, relevance screening, and evidence-strength classification are documented analytical steps; they do not change the original record count.

Leadership standard

Patient voice must become visible action.

The evidence layer is not the endpoint. It is the source discipline behind a practical PFAC operating model that can be assessed, used, measured, reviewed, and improved.
