Notion workspace • reviewed contribution process

Contribute evidence or implementation ideas without weakening source discipline.

The shared workspace is for public sources, de-identified improvement observations, benchmark notes, and practical toolkit ideas. Every contribution is reviewed before it is incorporated into the public APE materials.

What to submit

Four workstreams keep contributions organized.

1

Source Intake

Peer-reviewed articles, public reports, policy guidance, toolkits, and public datasets relevant to PFACs, patient experience, safety, equity, governance, or implementation.

2

Benchmark Notes

Publicly supportable examples of patient and family engagement practices at Dartmouth Health or comparable organizations.

3

Evidence Review

Comments on evidence type, strength, limitation, applicability, conflicting findings, or implications for leadership action.

4

Toolkit Backlog

Practical ideas for improving assessment, representation, action tracking, measurement, report-back, and Domain 5 traceability.

Review standard

What happens after submission.

Each item is checked for relevance, evidence type, source quality, duplication, claim strength, privacy, and whether it changes a practical leadership decision. Accepted sources remain distinguishable from implementation guidance, local hypotheses, and lived-experience material.

Is the source public and verifiable?
What does it support, and what does it not support?
Does it change a PFAC, patient-safety, experience, equity, or leadership decision?

Do not submit

No PHI, confidential records, restricted screenshots, or unsupported claims.

Use public sources and de-identified improvement observations only. Patient stories are treated as lived experience unless a separate evidence source supports the empirical claim.
