Turn patient and family input into accountable action.

This page defines the toolkit's purpose and the standard used across every tool. For the working operating agreement, use the separate Governance Charter.

Toolkit standard

What leaders should be able to do with this product.

Govern

Define the PFAC structure.

Document purpose, membership, sponsorship, meeting cadence, intake process, and decision rights.

Use the governance charter
Strengthen

Move beyond symbolic listening.

Connect patient and family input to action owners, timelines, metric signals, and follow-up communication.

Assess

Evaluate performance over time.

Review representation, influence, patient safety input, equity reach, leadership response time, and documented changes.

Patient-first guardrail

Every recommendation should pass a loved-one test.

PFACs should help leaders see what the system feels like to the person receiving care. The question is not whether the organization held a meeting. The question is whether patient voice changed design, behavior, communication, safety, or follow-through.

How does this benefit the patient or family?
How could this create risk if we get it wrong?
Would staff listen if the patient or family says something is wrong?
Would this be acceptable if the patient were me or someone I love?
